How do I schedule an appointment?
Dallas Surgical Group strives to provide the finest medical care for all patients by appointment only. You may schedule your appointment through one of our conveniently located offices listed on our CONTACT INFO section. Just give us a call today!

What are the office hours?
All of the Dallas Surgical Group locations’ office hours are Monday through Friday, 9:00 a.m. – 5:00 p.m. Please contact any of our locations to find out about our daily clinic schedules.

What health plans do you accept?
We participate with most major plans, please call our offices for more comprehensive lists. Some of our primary contracts are with:

What should I bring to my initial visit?
Dallas Surgical Group wants to make your initial visit registration process be smooth and efficient. Therefore, we ask that you bring your current insurance card, drivers license, referral form if applicable, medical history information pertinent to your medical condition, and any diagnostic studies. Additionally, you may complete your registration forms before your visit by simply printing them from the Patient Information section of this website or contact our office and our staff will mail you a copy.

Do I need a Referral from my PCP to be seen?
If you are insured under an HMO, POS, or EPO policy, in many cases you are required to secure a referral from your primary care physician (PCP) before being treated by a specialist. Make sure you understand and follow your plan’s rules carefully; otherwise your plan may reduce or deny payment of services. Prior to your appointment date, contact your PCP and verify whether a referral needs to be generated and have it faxed to our office or bring it the day of your visit.

Who do I call for after hour emergencies?
If encounter an after hour emergency, you may call the location you normally are seen at, and the answering service will page the physician-on-call. If you feel your medical condition cannot wait, please call 911 immediately.

How do I make a Refill Request?
Prescriptions of any type are best handled during regular clinic hours because your physician needs to check your medical record to verify the request. To receive the best service from us, we ask that you contact your pharmacy between 8:30 am and 4:30 pm, Mon.-Fri. In turn they will call us to authorize the prescription.

How do I learn of my Lab or Diagnostic Results?
Your physician or nurse will call you and provide your results over the phone. If necessary, we will request you make an appointment to come and discuss treatment options if a result necessitates this.

What is the practice’s Payment Policy?
As a courtesy to our patients, we will bill your primary and secondary insurance, including Medicare, for services rendered. Additionally, we will request you pay any plan-assigned co-payments, co-insurance amounts and deductibles at the time of service. We are always here to assist you, but we rely on you to provide us with current insurance information. When questions arise regarding your specific insurance coverage, we may look to you to assist us by you contacting your carrier to resolve any difficulties. If your insurance does not cover the full amount of the bill, you will receive a statement for the remainder of the charge. It is your obligation to pay this portion by the due date listed. If you believe your insurance provider handled your claim incorrectly, please contact your insurance provider directly.

May I have my medical records sent to a third party?
You may request that we release your medical records to a third party, such as an insurance provider or attorney. Before we release these records, you will have to sign a form authorizing us to release this confidential information. Please allow time for this to take place.

Will you fill out my disability insurance forms?
If you have an insurance policy that provides income to you while you are disabled, we can assist you with the forms. Before turning the forms into us, please complete the areas of the forms that you need to fill out. Our clinic will complete the remainder of the form and call you when it is ready. Lastly, there is a nominal fee for completing the forms.
